Young Adult Plan

Dr. West’s Young Adult Plan is for children of members, and other individuals, aged 16 – 25. With a focus on the health needs of young adults, this plan includes expanded access to telehealth and texting to fit the needs and expectations of today’s youth. It’s perfect for college students who want the continuity of their primary care physician at home.

Your all-inclusive membership fee includes all visits with me, plus the many benefits of concierge medical services:

Superior Access, Service and Convenience

• 24/7 access (including calling my cell phone for emergencies after hours and on weekends, and emailing or texting me directly for non-urgent questions)
• Convenient telehealth visits when appropriate
• Same-day or next-business day appointments and little-to-no wait time in the office
• Patient navigation services to help you schedule visits with specialists or diagnostics when needed
• Healthcare advocacy if hospitalized or for complex referrals (i.e., oncology, surgical)
• A state-of-the-art patient portal for easy reference to your health history

Strong Physician/Patient Relationship Focused on Prevention, Overall Health and Wellness

• Extended and unrushed visits, including:
  • 90-minute medical history review
  • 60-minute annual physical/wellness exam (in-person visit required)
  • Follow-up or urgent visits throughout the year
• Individualized, comprehensive health assessments with age-appropriate screenings such as cholesterol, diabetes, breast exam and pap smears, as well as counseling on others as needed (e.g., skin care, endocrine, sexual health, mental health, substance abuse)
• Comprehensive review, tracking, and administration of important vaccinations, including influenza, tetanus, HPV, and other catch-up vaccines as needed

Specialty Wellness Services

• Body composition and metabolic rate analysis
• Basic visual acuity and hearing screening
• For young women, routine gynecologic exams, evaluation of heavy or irregular cycles, screening for sexually transmitted infection, and contraception management
• Travel consults
• Access to discounted in-office labs and other office services

Membership fee: $2,500/year, or $1,750 if one parent is a member of the practice. The fee is payable annually. If not satisfied, you may cancel any time after the first six months simply notify us in writing with 30 days’ notice of your intent.
